Collect survey answers
Ask a cancellation or product question, receive the answer in your code and count choices in Analytics.
In Screens select New screen and choose the Cancel Survey template. It asks for a reason with the field reason and options like price, missing_feature and not_using. Edit the wording. To build your own add a Single choice and set Field ID and each Answer ID.
Publish it and point a placement such as cancel_survey at it.
Open it when the visitor starts cancelling:

Open Analytics. The Answers section shows each option with its share and visitor count. Text typed into a Text field is never sent to Miniwall: you receive it only in answers.
